In the most recent survey administered by the Nebraska Department of Health and Human Services, Southlake Village Rehabilitation and Care Center was rated deficiency-free.

It is unusual for a skilled nursing and care center to receive a deficiency-free survey, meaning all services provided at this care center meet or exceeded all state and federal standards. All skilled nursing and care centers are inspected annually to evaluate services and renew state licensure. Unannounced, surveyors come in to interview, observe, and check records for at least four days. If each of the hundreds of regulations are met, the skilled nursing and care center is awarded a “deficiency-free” survey.

Southlake Village Rehabilitation and Care Center is licensed as a skilled nursing facility serving individuals in need of short-term rehabilitation for reasons such as joint replacements, cardiac surgeries, stroke and a variety of other medical needs. As a skilled nursing and care center, Southlake Village Rehabilitation and Care Center also serves those needing long-term care and memory support care. Southlake Village Rehabilitation and Care Center is proud to serve its residents and patients with Registered Nurses 24 hours a day, as well as, the entire professional team.

Southlake Village Rehabilitation and Care Center in Lincoln, Nebraska is one of the 30 facilities owned or managed by the Vetter Health Services (VHS) group of Elkhorn, Nebraska. The care center’s administrator and team develop plans to continuously improve their services and facility with support from the VHS network. Operating decisions are made locally to ensure that Southlake Village Rehabilitation and Care Center remains responsive to the needs of its residents and community.
